What is an SAP evaluation and why is it required?
An assessment and clinical evaluation of an employee who works in a safety-sensitive position under the regulations of the Department of Transportation and who tested positive at work for alcohol or drugs. The assessment process will take place face-to-face, and will also include the following:
My recommendation for education and/or treatment
Referral to services with consideration for the employees resources and eligibility
Completing a thorough reporting and consultation with other professionals involved in the employee’s situation (i.e. MRO, DER, etc.)
Follow-up and Follow-through on employee’s documentation, paperwork, case management, and final meeting to determine compliance
What do I need to bring to the first appointment?
- Name and contact information for the Designated Employer Representative and/or Medical Records Officer who will receive/give information.
- Name and contact information for 2 other sources of information.
- A form of payment in money order, debit card, or credit card.
- Any legal documents stemming from incident
- BAC (blood alcohol content) breathalyzer results or urine toxicology report
- Any documentation from your job about this incident.
Can I just walk in to your office?
No. I have other services I offer at my office along with SAP services. I do make every effort to get you an appointment as soon as possible.
Do I have to do treatment?
Please keep in mind this is an individualized assessment and recommendation is made specifically for you. You will be referred to the appropriate level of care for your situation. Not everyone is referred for treatment.
What administrations or industries can fall under DOT regulations for federal drug and alcohol testing?
Organizations regulated by the Federal Motor Carrier Safety Administration (FCMSA), such as truck drivers operating Commercial Motor Vehicles with a gross vehicle weight rating of 26,001 pounds or more.
Organizations and employees regulated by the Federal Railroad Administration (FRA), including locomotive engineers, trainmen, conductors, switchmen, locomotive hostlers/helpers, utility employees, signalmen, operators, and train dispatchers.
Employees and crewman for aircraft with duties regulated by theFederal Aviation Administration (FAA), such as flight crewmember duties, flight attendant duties, flight instruction duties, aircraft dispatch duties, aircraft maintenance or preventive maintenance duties; ground security coordinator duties; aviation screening duties; and air traffic control duties.
Employees who operate or maintain revenue vehicles under theFederal Transit Administration (FTA).
Employees of Pipeline or Liquified Natural Gas (LNG) companies who perform operation, maintenance, or emergency-response functions regulated by the Pipeline and Hazardous Materials Safety Administration (PHMSA).
Persons who work on board vessels regulated by the United States Coast Guard (USCG) who hold a license, certificate of registry or merchant mariner’s document.
